When reporting the drivers who will be allowed to drive your Select-insured automobile, the following instructions from the Select application should be noted. A complete Select application package is available to anyone requesting a quote.
List all licensed drivers who are members of your household, regardless of whether they drive the automobile, are excluded from the policy, reside full-time where the covered automobile is garaged or reside in your home (e.g., students away at school).
Also include anyone who may be allowed to drive the automobile. Attach a separate sheet if necessary. On a separate sheet, list all accidents and violations occurring in the past five years for the above drivers.
Also include any accidents, violations or incidents which still appear on your driving record, regardless of when they occurred. List driver involved, incident date and description, amount of claim and whether or not your insurance company paid the claim.
Note: A person old enough to get a drivers license must be reported whether or not they have a license. Failing to report drivers will result in policy cancellation and denial of claims involving unreported drivers.
